To create your Veteran ID, we will need the following:

• Your DD-214 Form* (click for example)
• A valid photo ID, such as:
  • Driver's License

* Please make sure to white out or mask sensitive information, such as your social security number.

** Your DD-214 and ID will need to be in an image format such as JPEG, PNG, or GIF.

*** Your DD-214 image data will be destroyed after your application is approved or denied.
